Adrenocorticotropin (ACTH) is a polypeptide hormone secreted by the pituitary gland of vertebrate, which can promote the tissue hyperplasia of adrenocortical cortex and the production and secretion of corticosteroids. ACTH production and secretion are directly regulated by hypothalamus corticotropin releasing factor (CRH).
Overproduction of corticosteroids, in turn, can affect the pituitary and hypothalamus, reducing their activity. It can stimulate the development and function of adrenal cortex. It can stimulate the secretion of glucocorticosteroid. ACTH also regulates antibody production through the adrenal cortex. It's the opposite of auxin. ACTH measurement is mainly based on release and exemption method, including extraction method and direct method, which is simple and easy to be interfered by other substances.ACTH is unstable, easy to be degraded by protease at room temperature, blood samples are treated with EDTA anticoagulation, plasma is immediately separated and refrigerated, and frozen for several months.
The detection of corticotropin can be used in the diagnosis and differential diagnosis of hypophysis or hypoxia of the pituitary adrenal cortex. Corticotropin is valuable for differential diagnosis of hypoadrenal dysfunction and hypersecretion. In primary adrenal insufficiency, ACTH level is typically increased, while low ACTH level is usually seen in secondary adrenal insufficiency of pituitary dysfunction.ACTH test can also help identify the causes of excessive cortisol secretion in Cushing syndrome. When excessive cortisol secretion is caused by adrenal cortical lesions or hyperplasia, the ACTH level is typically decreased, while if it is caused by ectopic ACTH or excessive ACTH secretion in pituitary, the ACTH level is increased.
