Increase of Retinal Protein RBP3 Can Prevent Diabetes Patients From Developing Diabetes Retinopathy

Diabetes retinopathy (DR) is one of the most common complications of diabetes affecting more than 35% of patients. Endogenous protective factors may explain why only a small proportion of diabetes patients have diabetes retinopathy.
In a new study, researchers from Jocelyn diabetes Center, Harvard Medical School and other research institutions in the United States took people with type 1 diabetes for 50 years or more as subjects. More than 35% of these patients have no diabetes retinopathy or only mild diabetes retinopathy, and this has nothing to do with blood sugar control, which indicates that there are endogenous protective factors in some people.
Proteomic analysis of the retina and vitreous body identified that retinol binding protein 3 (RBP3), a retinol transporter mainly secreted by photoreceptors, was elevated in patients who were protected from late diabetes retinopathy. Mass spectrometry and protein expression analysis identified that the vitreous RBP3 concentration was negatively correlated with the severity of diabetes retinopathy.
Intravitreous injection of RBP3 and photoreceptor specific overexpression of RBP3 in rodents can inhibit the harmful effects of vascular endothelial growth factor (VEGF).
In terms of mechanism, these results indicate that recombinant RBP3 plays a therapeutic role by binding to and inhibiting tyrosine phosphorylation of VEGF receptor. In addition, by binding glucose transporter 1 (GLUT1) and reducing glucose uptake, RBP3 blocks the harmful effects of hyperglycemia on the induction of inflammatory cytokines in retinal endothelial cells and Miller cells. The up-regulated expression of RBP3 secreted by photoreceptors may play a role in the progression of diabetes retinopathy caused by hyperglycemia by inhibiting glucose uptake via GLUT1 and reducing the expression of inflammatory cytokines and VEGF.
